Key Responsibilities\n

As an LCSW at CHDC, your responsibilities will include:

• Delivering clinical behavioral health services aligned with the organization’s mission to achieve improved patient health outcomes and satisfaction.

• Guiding mental health consumers through the complexities of both physical and behavioral healthcare systems, ensuring they receive integrated care tailored to their needs.

• Educating and empowering individuals by informing them about available resources and services critical for disease prevention and promoting overall wellness.

• Actively participating in multi-disciplinary treatment team meetings, contributing your expertise to develop comprehensive care plans for patients.

• Timely reporting of any crises or unusual incidents to the Medical Director, and immediate notification of psychiatric and medical emergencies to the Chief Medical Officer (CMO).
\n
• Completing detailed documentation for patient records and other necessary reporting as directed.
\n
• Engaging in Quality Improvement programs, attending all relevant meetings and training sessions, and implementing new knowledge into practice.
\n
• Maintaining compliance with all organizational policies and procedures while working within CHDC’s integrated health care framework.
\n
• Administering evidence-based screening tools to assess the healthcare needs of individuals and diligently reporting assessment results.
\n
• Collaborating closely with our Center of Excellence Team to support patients dealing with Opioid Use Disorder, ensuring they receive comprehensive and compassionate care.

Patient Population / Client Focus\n

The predominant patient population comprises individuals seeking assistance with substance use disorders and related mental health issues. This role emphasizes the importance of providing culturally competent care while addressing the unique challenges faced by patients in a detox environment. Qualifications\n

To qualify for this role, candidates must meet the following requirements:

• A Master’s degree in Social Work is mandatory.

• Must hold a current Licensed Clinical Social Worker (LCSW) designation.

• A minimum of 2 to 3 years of relevant clinical experience in a behavioral health setting, preferably within a medical context.
