You will work remotely while delivering your services to patients within our Detox Center framework. Key Responsibilities\n

• Psychosocial Assessment: Conduct timely psychosocial assessments addressing the complexities of human behavior and psychosocial factors that could impact treatment outcomes. Maintain a deep understanding of the ethical and legal obligations, including identifying signs of abuse and adherence to patient rights.

• Clinical Intervention: Provide one-on-one counseling, family sessions, and group therapy aimed at mental health support and coping strategies. Facilitate family meetings to assist in decision-making processes involving hospice care, advance care directives, and other critical choices.

• Case Management: Manage a diverse clinical caseload while demonstrating the ability to prioritize high-risk situations, effectively balancing referrals and ongoing patient care. Collaborate closely with case managers to ensure seamless discharge planning.

• Documentation: Complete all required documentation that adheres to our policies, including social services progress notes and interdisciplinary plans of care, ensuring compliance and accuracy.
\n
• Education and Training: Participate in and lead in-service training sessions for hospital staff to promote awareness and understanding of psychosocial issues impacting patient care.
\n
• Quality Improvement: Engage in the development and analysis of process and outcome indicators to enhance patient care quality, assisting in continuous improvement initiatives within the social services department.

• Additional Duties: Manage special projects including Social Work Month activities, support groups, and educational programs tailored for patients, families, and staff to foster a supportive community.

Patient Population / Client Focus\n

This position primarily serves patients in the detoxification process, who may be dealing with substance use disorders and associated mental health challenges. You will work closely with individuals from diverse backgrounds, tailoring your approach to meet varied psychosocial needs and ensuring culturally competent care is provided.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• A minimum of 2 years of experience in a medical or healthcare setting, with a strong preference for at least 1 year of experience in a cancer-related environment.

Compensation and Benefits\n

The annual salary for this role ranges from $120,000 to $130,000, commensurate with experience and qualifications.
