Field-Based HIV Case Manager & Care Navigator

JWCH Institute, Inc. is seeking a Street Medicine HIV Case Manager in Los Angeles to deliver intensive, field-based case management, counseling, and care coordination for individuals living with HIV who are homeless or housing-insecure. You will support MCC and EHE goals by improving linkage to care, retention, and viral suppression, while tracking patients and coordinating across systems in dynamic field settings. #J-18808-Ljbffr JWCH Institute, Inc.
