Physical TherapistThis position organizes, coordinates, and delivers comprehensive physical therapy services to patients in the home care setting (home health and hospice for pediatrics and adults), ensuring care is effective, patient-centered, and aligned with best practices. Serves as a clinical specialist, maintaining advanced, professionally recognized expertise in the field, and remaining current with developments in physical therapy and home-based care. Applies this knowledge to enhance patient outcomes, support interdisciplinary collaboration, and share expertise with the broader healthcare community.
