Overview
Licensed Practical Nurse (LPN) – Long Term Care travel assignment in Geneva, New York, United States. Start date 09/15/2026 for 24 weeks. Weekly pay range: $1,303 to $1,373 per week. Guaranteed hours: 0.0. Shift: Rotating. Setting: Skilled Nursing Facility. This is a travel - temporary assignment.

Responsibilities
- Collect data relevant to residents’ physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team
- Contribute to a developmentally appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions, involving the resident/family and healthcare team
- Recommend revisions to the plan of care after evaluating resident outcomes; identify referrals to address all aspects of care
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse; collaborate with physicians and ancillary staff; maintain a safe, healthy environment
- Maintain current knowledge and competency in nursing practice; participate in mentoring or orientation of staff; engage in continuing education and inservice programs
- Demonstrate ability to care for geriatric residents and adapt care to adult and geriatric patient needs

Requirements
- Active New York LPN license
- BLS certification (AHA or RQI required)
- Minimum 1 year of experience as an LPN, with prior LTC exposure preferred
- Ability to provide care to adult and geriatric residents
- Willingness and ability to work rotating shifts
- Capable of meeting facility policies and performing duties in a skilled nursing setting
