Pediatric Transitional NursePediatric Transitional Nurse – Registered Nurse (RN) / Licensed Vocational Nurse (LVN) Pasadena, TX | Bilingual Preferred (Not Required) | Full Time Pay Range:LVN: $30–$35/hourRN: $35–$42/hourMake a Difference Beyond One Shift Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Vocational Nurses (LVN) for our Pediatric Transitional Nurse Team to support medically fragile children and their families during critical transitions to home care. This role is ideal for nurses who are confident working with high-acuity patients and skilled in trach, vent, G-tube, and emergency management. Nurses in this position play a critical role supporting families, training caregivers, and ensuring safe continuity of care in the home environment. The company was founded by a nurse with a huge heart, and today we pride ourselves on our values: H eart, A dvocacy, L ove, and O utreach! #HALOS What You'll DoProvide skilled pediatric nursing 1:1 care in patients' homesPerform patient assessments and ongoing evaluations -Administer medications, treatments, and therapiesSupport hospital-to-home transitions for high-acuity patientsEducate and support parents, families, and caregiversAssist with discharge support and care coordinationTrain and support permanent field nurses during transitionsProvide first-shift and interim coverage as neededCollaborate with Clinical Care Managers and interdisciplinary teamsRespond appropriately in emergency situations and changes in patient conditionQualifications Required:Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) licenseAt least 1 year post license certificationCurrent CPR certificationAbility to lift and assist pediatric patients as neededAt least 3 years of experience with Trach care, Ventilator management, and TPN, post-graduationEmergency response managementPediatric home health experienceHigh-acuity pediatric patientsSchedule & Travel12 hour shifts (Day or Night)Flexible scheduling based on patient needs and Nurse availability5 working days per week, minimum 1 weekend dayOn call and Staffing coverage responsibilities as neededLocal travel within approximately 30–40 milesWhy Angels of Care:We have a full benefits package, bulleted out below.
