What You'll Do:

• Follow a full panel of hematology-oncology patients with complex care needs
• Conduct treatment visits, follow-ups, and some new patient assessments
• Coordinate care plans for patients with co-morbidities
• Collaborate with physicians and multidisciplinary team
• Work in outpatient clinic setting with infusion suite support
• Ensure seamless patient care, including managing appointments and treatments
• Rotate call responsibilities on Saturdays approximately once every 3 months

What You Need:

• Valid Nurse Practitioner license in Louisiana
• 3-5 years of hematology-oncology experience
• Strong clinical assessment and care coordination skills
• Ability to work independently and as part of a team
• Excellent communication skills
