Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to develop formulations and tailor treatment plans for each individual based on their needs.

• Implement and utilize evidence-based therapeutic approaches, including solution-focused therapy, behavioral activation, motivational interviewing, and other relevant modalities to enhance patient outcomes.

• Collaborate effectively with a multidisciplinary team that includes psychologists and primary care physicians to provide integrated care for patients.
\n
• Monitor patient progress, making adjustments to treatment plans as necessary, and document all sessions accurately in a timely manner.

• Provide education and support for the management of psychotropic medications, collaborating closely with prescribing medical providers to ensure adherence and understanding of treatment.

Patient Population / Client Focus\n

The LCSW will primarily work with adult patients in a hospice setting, where understanding the complexities of end-of-life issues is paramount. You will support patients and their families as they navigate emotional challenges associated with terminal illness, grief, loss, and transitions. Qualifications\n

• A Master’s degree in Social Work from an accredited program is mandatory.

Compensation and Benefits\n
