Key Responsibilities

The Home Care Physical Therapist will be responsible for a variety of essential duties, including but not limited to:

• Conducting comprehensive evaluations of patients in their home environment to assess mobility, pain levels, and overall functional capabilities.
• Developing customized treatment plans that incorporate therapeutic exercises, hands-on therapy techniques, and assistive equipment tailored to the patient's specific needs.
• Implementing effective interventions and continually monitoring progress, adjusting goals and treatment strategies as necessary to ensure optimal recovery.
• Documenting patient treatments, responses, and progress in accordance with regulatory requirements and best practices.
• Educating patients and their families on physical therapy modalities, self-management strategies, and the safe use of assistive devices.
• Staying current with advancements in the field through professional development, including workshops, seminars, and networking with peers in the industry.

Patient Population / Client Focus

The Home Care Physical Therapist will serve a diverse clientele that includes children, adults, and seniors dealing with various physical challenges related to recovery from surgeries, injuries due to falls or accidents, or chronic conditions that impede daily function. By meeting patients where they reside, you'll be able to observe firsthand the barriers they face and work collaboratively with them to achieve their rehabilitation goals.

Qualifications

To excel in this role, candidates must meet the following qualifications:

• Hold a Doctorate in Physical Therapy (DPT) and possess a valid physical therapy license in the State of Florida.
• Demonstrate strong communication skills, both written and verbal, to effectively share treatment plans and educate patients and families.
• Possess basic computer skills, including proficiency in documentation software and electronic medical records.

Preferred Experience

While a minimum of two years of experience as a Physical Therapist is required for this position, candidates with a willingness to learn, especially recent graduates, will also be considered.
