Work Environment

The role is predominantly onsite, as you'll be visiting patients in their homes across the Fort Myers area. You will work collaboratively with other healthcare professionals and your colleagues at North West Physiotherapy to ensure comprehensive care delivery. This dynamic environment requires adaptability and excellent communication skills, as you will be interacting directly with patients and family members in various home settings.

Key Responsibilities

• Conduct timely evaluations of patients within twenty-four (24) hours of receiving a physician referral, documenting any delays clearly in medical records.
• Develop individualized treatment plans by collaborating with referring physicians and advocating for necessary services.
• Implement treatment protocols effectively, ensuring adherence to the physician's directives while modifying approaches based on patient responsiveness.
• Supervise and mentor Physical Therapy Assistants (PTAs) during patient care activities, providing guidance in line with state practice laws.
• Regularly communicate with supervisors and interdisciplinary team members regarding patient progress, challenges, and future treatment plans.
• Engage in Patient Care Conferences and Rehabilitation Meetings to discuss care strategies and outcomes.
• Participate in ongoing education by conducting in-service training for facility staff and promoting evidence-based practices.
• Maintain accurate documentation according to PT Board regulations, including evaluations, treatment modalities, and patient progress.
• Instruct patients and their families on home exercise programs and safety measures in preparation for therapy discharge.
• Assist with discharge planning, ensuring patients have the resources needed for continued recovery post-therapy.
• Recommend and facilitate the acquisition of durable medical equipment, enabling patients to regain mobility and independence.
• Report equipment issues promptly to maintain a safe and effective treatment environment.

Patient Population / Client Focus

You will work with a diverse range of patients, from post-operative individuals to those managing chronic conditions and recovering from injury. The home care setting allows for personalized treatment in a familiar environment, promoting better engagement in the therapeutic process. Your ability to connect with patients and their families is essential for fostering trust and cooperation.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• Valid physical therapist license in the State of Florida.
• Minimum of 2 years of experience as a physical therapist, preferably in a home care or outpatient setting.
• Exceptional communication skills to effectively engage with patients, families, and healthcare team members.
• Strong organizational and time management skills to handle a busy caseload efficiently.
• Self-motivated with a proactive approach to problem-solving and patient care.
• A compassionate mindset, dedicated to providing quality patient care.

Preferred Experience

• Experience in outpatient orthopedics and familiarity with manual therapy techniques and sports rehabilitation.
• Advanced knowledge in therapeutic modalities and evidence-based practice in physical therapy.
• Prior experience training support staff or conducting in-service educational programs.

Compensation and Benefits

The salary for this role ranges from $90,000 to $100,000 per year, commensurate with experience and qualifications.
