What You'll Do
- Evaluate the physical therapy needs of assigned patients in the home health setting
- Develop and implement individualized treatment plans to improve mobility and independence
- Assist with patient transfers and positioning as needed
- Document care activities accurately in electronic medical records promptly
- Communicate effectively with patients, families, physicians, and care staff
- Provide age-appropriate, safe, and effective care for pediatric to geriatric patients
- Ensure compliance with all healthcare regulations and facility policies
What You Need
Required:- Valid Illinois Physical Therapist license
- Prior experience in outpatient or home health settings
- Ability to transfer and position patients safely
- Experience working with diverse age groups, including pediatric and geriatric populations
