Key Responsibilities\n

• Facilitate access to critical community resources, including support groups, vocational rehabilitation services, and housing assistance, ensuring patients receive comprehensive care during and after their hospice experience.

• Educate patients and families about available community resources and post-rehabilitation services, enhancing their ability to transition effectively from hospice care back to their communities.
