Importance of This Role

As a Home Health Physical Therapist , you play an essential role in the care team, assisting patients in their recovery, enhancing mobility, and preserving their independence in the environment they feel most at ease: their home.

This position offers a high degree of autonomy and is centered around building relationships, allowing you to witness the tangible effects of your efforts daily.

Your Responsibilities

• Deliver Patient-Centered Care:
  • Assess and evaluate patients’ functional abilities, mobility, strength, and rehabilitation potential.
  • Create and execute personalized care plans in partnership with physicians and the care team.
  • Administer hands-on therapy to enhance mobility, strength, balance, and independence.
  • Educate patients and their caregivers on treatment protocols, exercises, and safety measures.
• Drive Outcomes & Recovery:
  • Track patient progress and modify treatment plans as necessary.
  • Facilitate safe transitions of care and assist in discharge planning.
  • Identify additional patient needs and collaborate with interdisciplinary teams.
• Collaborate & Communicate:
  • Work closely with nurses, therapists, and physicians to ensure cohesive care.
  • Accurately document patient care and progress in the electronic medical record (EMR).
  • Engage in case conferences and team meetings.
• Lead & Support:
  • Oversee Physical Therapist Assistants (PTAs) in line with state regulations.
  • Provide guidance and education to caregivers and support personnel.

Qualifications and Skills

• Graduation from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• 1–2 years of experience preferred, particularly in home health.
• Strong clinical assessment and critical thinking capabilities.
• Excellent communication skills and ability to educate patients.
• Current CPR certification.
• Valid driver’s license, dependable transportation, and auto insurance.
