Position Overview

Body Balance Rehab is actively seeking dedicated Physical Therapists to join our compassionate and dynamic team in Rancho Cucamonga, California. This full-time position will primarily focus on providing in-home care to clients, helping them improve their mobility, function, and overall quality of life through personalized therapy solutions.

Work Environment

This position entails providing physical therapy services onsite, directly in our clients' homes. You will work directly with patients in a familiar environment, allowing for a more comfortable and effective therapy experience. Our organizational culture promotes collaboration and openness, with full administrative support to facilitate a focus on patient care over paperwork. The standard working hours will be Monday to Friday from 8 AM to 3 PM, allowing for a balanced work-life dynamic.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations for clients, focusing on identifying individual needs and challenges.
• Develop, implement, and adjust personalized treatment plans tailored to the unique requirements of each patient.
• Regularly reassess clinical signs, symptoms, and progress towards rehabilitation goals, modifying treatment plans as necessary.
• Document all therapy sessions meticulously, including SOAP notes, progress reports, and ongoing care plans through our electronic medical record system.
• Instruct and counsel clients and their families regarding rehabilitation processes, setting realistic expectations and addressing challenges faced during recovery.
• Meet or exceed established productivity targets and other business objectives as stipulated by the organization.
• Collaborate with fellow interdisciplinary team members to enhance care for clients receiving multiple services.
• Uphold the company's values and culture in every interaction and within the community, acting as an ambassador for Body Balance Rehab.

Patient Population / Client Focus

You will work with a diverse range of patients, including children and adults with varying medical diagnoses, backgrounds, and treatment needs. Whether you are conducting assessments for a young child recovering from a sports injury or aiding an elderly client post-surgery, your role will be crucial in fostering better health outcomes and independence.

Qualifications

• A Bachelor's Degree in Physical Therapy is required, and preference will be given to candidates with a Master's or Doctor of Physical Therapy (DPT) degree from an accredited program.
• Current and unrestricted state licensure from the Board of Physical Therapy to practice in California.
• Ability to perform physical therapy evaluations and treatments both face-to-face and using telehealth modalities as clinically appropriate.
• Passion for working with a diverse client population, demonstrating cultural competency across age, race, and ethnicity.
• Flexibility and adaptability, essential for navigating the dynamic healthcare landscape we operate in.
• Strong team collaboration skills paired with a customer-focused approach.

Preferred Experience

• Prior experience in home health settings is highly desirable but not mandatory.
• Experience with a variety of therapeutic practices and knowledge of specialized rehabilitation techniques.

Compensation and Benefits
