Work Environment

This role will primarily be conducted in patients' homes, allowing therapists to provide personalized, one-on-one care in a comfortable and familiar setting. Key Responsibilities

• Conduct thorough evaluations of each patient's condition, including gathering pertinent medical history, performing specialized assessments, and documenting results to establish a tailored treatment plan.
• Engage with patients and their families to explain evaluation findings, treatment recommendations, and goal-setting processes to foster an understanding and collaborative environment.
• Implement individualized treatment protocols based on evaluation insights, current patient health status, and industry best practices to optimize patient recovery outcomes.
• Collaborate with other healthcare professionals as necessary, ensuring that any changes in a patient's condition are recognized and addressed through appropriate modifications to the care plan.
• Effectively manage discharge planning and follow-up care to ensure continuity of treatment, and provide accurate documentation of the patient's care journey in compliance with evolving healthcare regulations.
• Exhibit strong time management skills, meeting productivity expectations while maintaining a punctual schedule for both patient appointments and team meetings.
• Contribute to departmental goals and objectives, participating in marketing initiatives and community outreach efforts aimed at enhancing service visibility and accessibility.
• Proactively pursue continuing education opportunities to further enhance professional competencies, while also serving as a clinical instructor to students in training.
• Demonstrate flexibility and adaptability in delivering patient care to ensure an excellent customer experience and improved accessibility to services.

Patient Population / Client Focus

As a Physical Therapist at Advanced Injury Treatment Center, you will work with a diverse patient population, including individuals recovering from surgery, injuries, and chronic conditions. Qualifications

• Bachelor's degree in Physical Therapy or related field.
