Work Environment\n

This position offers the flexibility of remote work, allowing you to integrate your professional commitments with your personal life effectively. The work schedule is Monday to Friday from 8:00 AM to 4:30 PM, with potential for occasional evening or weekend hours to accommodate client needs.

Key Responsibilities\n

• Conduct thorough assessments of seniors' psychosocial needs, considering emotional, mental, and social factors, while also evaluating their living conditions.

• Collaborate with clients and their families to develop personalized care plans tailored to their specific goals, preferences, and desired outcomes.

• Provide counseling and emotional support to seniors, guiding them through life transitions such as health issues, loss of independence, and family dynamics.

• Advocate for clients' rights and help them gain access to essential community resources, ensuring they receive the necessary support and services.

• Work in conjunction with nursing staff, caregivers, and other healthcare professionals to coordinate care and optimize the outcomes for each client.

• Conduct regular check-ins with clients to monitor their well-being and address any emerging issues related to their care or social engagement.

• Educate clients and their families about available community programs, support groups, and resources to enhance their overall quality of life.

• Maintain accurate records of client assessments, individualized care plans, and progress notes using electronic health record (EHR) systems.

• Stay informed on the latest social work practices and legislative changes to ensure compliance with ethical standards and improve service delivery.

Patient Population / Client Focus\n

Your primary clientele will be seniors experiencing various psychosocial issues. You will be integral in addressing their mental health needs, family dynamics, and social isolation, often in partnership with their family members and caregivers. Building rapport and trust with clients is essential to effectively support their transitions and challenges.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• A current and valid state licensure as a Master Social Worker (LMSW).

• At least two years of experience in a social work role, preferably in a community center or geriatric setting.

• Deep understanding of aging-related issues, social work principles, and community resources.

Compensation and Benefits\n
