About the Facility Home Health Radius rule: Traveler's permanent address must be at least 50 miles outside of the home unit Department Information Visits will be conducted throughout University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, Huntersville, and surrounding areas No maximum mileage limit; average daily driving is 60-80 miles. Mileage is reimbursed weekly Ideal central housing locations include University City and/or Concord, with flexibility to travel throughout the listed areas Team members are required to complete 6 visits per day, including routine visits, discharges, return of care (ROC), recertifications, and reassessments Minimum experience required: 1 year in Home Health Nursing First-time travelers are accepted if experienced in home health Patient Types and Common Diagnoses Chronic diseases Wound care and wound vac IV therapy PleurX management Licenses, Certifications, and Position Requirements Valid Driver's License and proof of insurance BLS certification Required Skills Experience with Homecare Homebase charting (mandatory, no exceptions) Proficiency in IV therapy, PleurX, wound vac, wound care, patient assessment, disease teaching, OASIS documentation Case management skills, with the ability to manage 25-30 patient episodes simultaneously Department Support Departmental leadership Scheduler Clinical Manager Clinical Supervisor Peers and clinical team Technology and Equipment EPIC Rover (view only access) Focura for uploading discharge instructions and important documents such as wound photos Samsung Note 9 phones Homecare Homebase (experience required) Orientation New Employee Orientation (NEO) Five-day orientation with a preceptor Scheduling No weekend rotation unless the traveler opts in Call is not required but may be available if interested Travelers are expected to work all holidays unless excused by management Return to Office (RTO) requires manager approval Schedule cycles every 13 weeks Additional Information Scrub color: Navy blue Free parking available Department culture emphasizes a team environment Additional Notes Appointment scheduling: Visits are scheduled with patients the day before the planned visit Cancelled appointments do not count as visit equivalents; 30 visit equivalents are required weekly No company car is provided Clinic days vary by branch, typically one in-person day per week and occasional virtual days A company phone is provided for documentation and communication with patients and providers Shift Requirements Minimum 1 year of Home Health Nursing experience BLS certification Valid Driver's License and proof of insurance Experience with Homecare Homebase EMR (mandatory) #J-18808-Ljbffr
