What We're Looking For The Care Manager - 1915i is responsible for providing complex care coordination and monitoring to members having a primary Intellectual or Developmental Disability, which may include a secondary mental health or substance abuse, and/or complex medical/physical health need. This position requires a dynamic, proactive approach to assessment, monitoring and complex coordination of care, to ensure quality support and consistent adherence to waiver requirements. The Care Manager - 1915i assists with integrated care management through multi-disciplinary collaboration. On a Typical Day, You Might Utilize person-centered planning methods & strategies to gather information, to get to know the members supported and to adequately address specific needs of the individual. Provide education and support to members on learning about and exercising rights and responsibilities. Complete discovery activities (information gathering and assessment) as a component of the planning process. Ensure that assessments and plans are updated as needed and whenever the member’s life circumstances change. Employee Benefits Trillium knows that work/life balance is essential.
