Key Responsibilities\n

• Conduct thorough, culturally sensitive bio-psychosocial assessments to identify mental health, substance abuse, and medical needs within a primary care framework.

• Deliver evidence-based behavioral health services to individuals, couples, and families. This includes counseling, intake sessions, diagnoses, treatment planning, crisis interventions, and referrals to substance use disorder treatment.

• Maintain accurate electronic documentation of patient evaluations, progress notes, and suicide risk assessments to monitor and evaluate patient responses to treatment.

• Serve as a liaison between patients and community resources, helping to coordinate support services, including medication assistance programs and linkage to local agencies.

Qualifications\n
