CircleLink Health is hiring a remote RN Care Coach to support Medicare patients with 2+ chronic conditions on a part-time basis (about 20–25 hours per week). You will educate, coordinate care, and coach patients to close preventive gaps while helping them manage at home. You will document thoroughly and manage timelines with strong organization and initiative.
