Registered Nurse (RN) – Case Management

Duration: 13 Weeks

We are seeking an experienced Case Management Registered Nurse (RN) to coordinate patient care, facilitate discharge planning, and support utilization management activities within an acute care hospital setting. The ideal candidate will have strong care coordination skills, acute care experience, and proficiency with Epic.

• Day Shift
• 5 x 8-Hour Shifts
• Shift times may vary:
  • 7:00 AM – 3:30 PM
  • 8:00 AM – 4:30 PM
• Weekend rotation required

• Minimum 1 year of Case Management and/or Acute Care RN experience required
• First-time travelers accepted
• Acute Care experience required
• Epic experience required

• Active Alaska RN License required
• BLS Certification

• ACM (Accredited Case Manager) Certification

• Coordinate patient care throughout the continuum of care
• Facilitate discharge planning and transition planning
• Collaborate with physicians, nurses, social workers, and interdisciplinary teams
• Assess patient needs and identify appropriate resources
• Support utilization management and length-of-stay initiatives
• Advocate for patients and families to ensure optimal outcomes
• Maintain accurate and timely documentation within Epic
• Assist with complex case coordination and care planning
