Key Responsibilities

• Conduct thorough evaluations of patients in their home environments to understand their specific needs and therapy goals.
• Develop individualized treatment plans based on clinical assessments and patient discussions to facilitate optimal rehabilitation.
• Administer various treatment methods including therapeutic exercises, manual therapy techniques, and the use of specialized equipment to alleviate pain and promote healing.
• Monitor and document patient progress meticulously, adjusting treatments accordingly to improve patient outcomes.
• Educate patients, caregivers, and family members on treatment plans, self-care strategies, and home exercises, fostering a supportive recovery environment.
• Stay current with industry best practices by attending workshops, engaging in professional societies, and reviewing relevant literature.

Patient Population / Client Focus

You will work with a diverse patient population primarily consisting of individuals recovering from surgeries, injuries, or chronic conditions. This role focuses on family-centered care, ensuring that patients feel supported and empowered throughout their rehabilitation journey. You will address various conditions—from post-operative orthopedic cases to neurological impairments—enabling patients to increase their functionality and regain independence in daily activities.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current and valid State License in Physical Therapy.
• Excellent written and verbal communication skills to effectively interact with patients, families, and the healthcare team.
• Proficiency in basic computer skills, with the ability to document treatment plans and patient progress on electronic systems.
• Demonstrated adaptability, flexibility, and a positive attitude towards patient engagement.

Preferred Experience

• While prior experience is advantageous, we welcome new graduates who show passion, commitment, and a desire to learn. A background in home healthcare settings is a plus.
• Experience working with assistive devices and knowledge of therapeutic modalities is beneficial.

Compensation and Benefits
