Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, patient observations, and the review of medical records to identify factors influencing patients' emotional and social recovery.

• Provide counseling and therapeutic support to both patients and their families, addressing challenges related to adjustment, coping mechanisms, and emotional well-being.

• Collaborate actively with rehabilitation specialists to create individualized treatment plans that incorporate both psychosocial and medical perspectives, ensuring a comprehensive approach to care.

• Assist patients in accessing necessary community resources, such as support groups, vocational rehabilitation services, or housing assistance, to facilitate their continued progress.

• Educate patients and families about available community resources and post-rehabilitation services to promote successful transitions post-discharge.

• Engage in discharge planning activities, ensuring a smooth transition for patients returning home or moving to alternate care settings, while offering ongoing guidance and support.

Patient Population / Client Focus\n

You will work with a diverse array of patients in our hospice center, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from USD 75,000 to 100,000 annually, in addition to a sign-on bonus.
