Position Overview

Paramount Physical Therapy Ltd. is actively seeking a dedicated and experienced Physical Therapist (PT) to join our team at our state-of-the-art rehabilitation center located in Freehold, New Jersey. In this full-time onsite role, you will have the responsibility of providing exceptional care to our patients, empowering them to improve their physical well-being through tailored treatment plans and evidence-based techniques.

Work Environment

You will be working in a vibrant rehabilitation center that is well-equipped with modern therapy technologies and treatment facilities. Our supportive atmosphere encourages open communication and teamwork among staff members. Operating from 8:00 am to 5:00 pm, Monday through Friday, your role will allow for a stable schedule that fosters work-life balance.

Key Responsibilities

• Assess and treat individuals with functional limitations and disabilities, carefully examining their needs and developing comprehensive treatment plans supported by clinical evidence.
• Conduct evaluations that include gathering pertinent medical histories and performing specialized assessments to guide the rehabilitation process for each patient.
• Implement treatment protocols tailored to individual evaluations, adapting strategies based on patient progress and evolving healthcare standards.
• Maintain proactive communication with interdisciplinary healthcare teams, documenting any changes in patients’ conditions that may affect their care plans.
• Complete thorough discharge planning and follow-up strategies, ensuring seamless transitions for patients as they progress.
• Document all patient interactions and treatments accurately and in a timely manner, including daily notes and progress summaries, to maintain high-quality care records.
• Participate in continual education and training initiatives to stay compliant with state regulations and enhance your clinical competencies.
• Engage in facility meetings and contribute to initiatives focused on increasing patient access to care, including understanding data metrics relevant to our service areas.

Patient Population / Client Focus

Your primary focus will be on outpatient orthopedic patients who require skilled physical therapy intervention to recover from injuries or surgeries. You will work closely with individuals looking to regain strength, mobility, and independence, making your role pivotal in their rehabilitation journeys.

Qualifications

• Minimum requirement of a Doctorate of Physical Therapy (DPT) from an accredited program.
• Active physical therapy license in the state of New Jersey.
• Two (2) years of hands-on experience in outpatient orthopedic settings, demonstrating a strong foundation in patient care.
• Current CPR certification to ensure safety and preparedness in emergency situations.

Preferred Experience

We prefer candidates who have a background in collaborative team environments and have experience employing manual therapy techniques and modalities to facilitate recovery. Familiarity with electronic health records and rehabilitation technologies will also benefit your practice in our facility.

Compensation and Benefits
