Key Responsibilities\n

• Conduct thorough clinical assessments to evaluate patients' psychological, emotional, and social needs, and develop tailored treatment plans.

• Maintain accurate and timely clinical documentation that complies with state and federal guidelines, ensuring the highest standard of care.

• Collaborate with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals, to create comprehensive care strategies for patients.

• Provide education and support to patients and their families, encouraging participation in their own care and promoting mental well-being.

• Remain current on advancements in mental health practices, treatment modalities, and legislative changes, ensuring optimal care delivery.

Patient Population / Client Focus\n

As an LCSW at our hospice center, you will work with a diverse range of patients, including those facing chronic illnesses, terminal conditions, and end-of-life challenges. Your expertise will serve as a vital resource for patients and their families, allowing you to facilitate discussions around grief, coping strategies, and alternative therapies to improve their emotional and psychological well-being.

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
