Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through in-depth interviews, patient observations, and thorough medical record reviews to develop a holistic understanding of each patient’s emotional, social, and environmental factors influencing their recovery journey.

• Provide empathetic counseling and emotional support to patients and their families, helping them navigate adjustment challenges and enhance their mental well-being throughout the recovery process.

• Collaborate with the rehabilitation team to establish tailored treatment plans, with a focus on addressing patients' psychosocial needs and aligning with their recovery goals.

• Facilitate access to critical community resources, such as support groups, vocational rehabilitation programs, housing assistance, and post-rehabilitation services, ensuring patients have the necessary tools to thrive post-discharge.

• Contribute to discharge planning, assisting patients in the safe transition back to home or alternative care settings, while ensuring they have a robust support network in place.

Patient Population / Client Focus\n

The patients you will assist primarily include individuals in detoxification from substance use, as well as those dealing with co-occurring mental health disorders. This diverse population often requires emotionally sensitive, innovative approaches to overcome barriers related to substance dependence and mental health challenges, underscoring the importance of your role in their recovery trajectory.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution, ensuring a strong foundation in theoretical and practical aspects of social work.

Compensation and Benefits\n
