Lead Registered Nurse (RN) - Transitional Care Unit - $36-58 per hour

Position Overview Team Lead/Charge – Transitional Care Unit (RN) Location: Valley Park, Missouri Employment Type: Staff (Ongoing) Responsibilities Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into the ambulatory care setting. Critically assesses patient data to proactively mitigate impending changes in condition and establishes consults to the inter‑professional team as appropriate. Leads the development and implementation of complex individual plans of care with defined goals in collaboration with the inter‑professional team and the patient, family, or caregiver. Plans of care reflect application of expert holistic clinical knowledge and skill to anticipate and prioritize complex care needs, anticipating barriers and incorporating inter‑professional and family input. Actively utilizes and champions scientific inquiry for evidence‑based practices (e.g., participates in journal club, facilitates practice change, recommends new standards of care, conducts literature searches). Explores opportunities to advance nursing practice (e.g., community service health events, patient/family education groups, ethics advisory committees, adjunct faculty roles, leadership board participation). Analyzes gaps in practice standards and provides feedback to peers in collaboration with leadership to enhance safe patient care. Leads the evaluation of the patient and family experience and adjusts care provided to improve patient satisfaction and quality of care. Qualifications Registered Nurse (RN) license required. 2–5 years of nursing experience, preferably in the Emergency Department (ED). BSN preferred due to Magnet status; Bachelor’s degree in Nursing and 5–10 years of experience considered preferred. No supervisor experience required.
