Position Highlights Excellent career growth opportunity $10,000 Sign-On Bonus Eligible* Additional $5.00 Med/Surg Shift Differential 12 hour Night Shifts 7:00 pm to 7:00 am Weekend, Holiday Requirement No Call 2 years of RN work experience needed Benefits eligible first day of month after start 401k with Match/Pension Overview Acute Care Medicine manages patients with a full range of clinical issues—from significant blood dyscrasias, advanced endocrine pathology, heart and renal failure, GI conditions, infectious diseases, asthma, cystic fibrosis, and a multitude of other conditions. The multidisciplinary team includes physicians, nurses, advanced practice nurses, case managers, social workers, physical therapists, dietitians, and pharmacists. The unit is supported by a clinical manager and lead charge nurse, and is responsible for providing quality patient care through coordination of interdisciplinary collaboration. Responsibilities Facilitate identification and resolution of gaps in care from hospitalization, through discharge, and into ambulatory care. Critically assess patient data to proactively mitigate impending changes in condition and establish consults to the interprofessional team as appropriate. Lead the development and implementation of complex individual care plans with defined goals, collaborating with team members and the patient or family. Utilize and champion evidence‑based practices, participating in journal clubs, facilitating practice change, and disseminating new knowledge. Explore opportunities to advance nursing practice, including community health events, patient/family education, community advisory boards, and adjunct faculty roles. Analyze gaps in practice standards and provide feedback to peers and leadership to enhance safe patient care. Lead the evaluation of the patient and family experience, adjusting care to improve satisfaction and quality of care. Work in a constant state of alertness and maintain safety as a safety‑sensitive position.
