Description

• Assess patients in either inpatient or outpatient settings and create personalized treatment plans
• Apply physical therapy interventions that target mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Guide patients and their families on therapeutic goals, planning for discharge, and suggested home exercise routines
• Maintain accurate documentation of evaluations, treatments, and progress according to facility and regulatory standards

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited educational institution

Certifications

• Valid state licensure as a Physical Therapist
