Our RN Case Managers play a critical role in coordinating care, building meaningful patient relationships, and delivering outcomes that truly make a difference.Description:Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.Education/License:Diploma from an accredited school/college of nursing. Required professional licensure at time of hire.Responsibilities:Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. Performs direct participant care, including acute triage, blood draws, wound care, etc.Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care. Serves as a primary coordinator of all disciplines for well-coordinated patient care.Carries out physician and nursing orders. Attends team meetings and care plan meetings.Assesses and coordinates patients' discharge planning needs with members of the healthcare team. Participates, develops, and provides education to participants, caregivers, and associates.Participates in after hours/weekend on-call rotation, as needed. May require travel to participant place of residence, long term care and assisted living facilities.Other duties as assignedThe payrate for this position is from $39.00 - $54.12 per hour.
