Key Responsibilities\n

• Initiate discharge planning from the moment each patient is admitted, assessing their needs and preparing them for a successful transition post-treatment.
\n
• Conduct comprehensive discharge sessions to empower patients with coping strategies and ensure they understand their transition plans and available resources.
\n
• Confirm discharge appointments with patients and their families, ensuring they are fully informed and prepared for the next steps in their care.
\n
• Communicate effectively with referral sources and outpatient providers to promote continuity of care at all stages: admission, midpoint, and discharge.
\n
• Facilitate transitions of care to higher levels of service when clinically necessary, advocating for patient needs and maintaining open lines of communication with involved parties.
\n
• Participate in outreach initiatives and clinic tours, representing our services to the community and enhancing public awareness of mental health resources.
\n
• Address and manage crises effectively during sessions, employing de-escalation techniques and providing immediate support as required.
\n
• Collaborate with milieu staff to enhance patient experiences and support their interactions with loved ones, fostering a positive and nurturing environment.
\n
• Maintain participation in multidisciplinary team meetings, contributing insights and updates on patient progress and care strategies.
\n
• Conduct thorough risk assessments, evaluating patients for suicidality and any potential danger to themselves or others, alerting the team as necessary.
\n
• Provide interim clinical support for other clinical staff as needed, ensuring all patient care responsibilities are seamlessly covered.

• Stay in close contact with Utilization Review and Business Departments regarding patient authorizations and billing, ensuring transparency and efficient service delivery.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or another related mental health discipline is required.

• Possession of an independent California license as a Licensed Clinical Social Worker (LCSW) is mandatory.
