Geriatric Social Work Lead: Care Coordination & Mentorship

On Lok PACE is seeking a Social Work Lead to provide direct case management and act as SME for the IDT, overseeing assessments, care planning and referrals to community resources. This role supports participants to maintain independence and navigate complex social needs within the PACE model. Under supervision from the Regional Social Work Manager and Program Manager, you will mentor staff, ensure compliance, and contribute to continuous quality improvement of social work services at the center. #J-18808-Ljbffr On Lok Inc.
