Responsibilities
- Administer prescribed medications and treatments, monitor responses, and report changes to the RN or physician as appropriate.
- Perform and document routine assessments, vital signs, and relevant observations; identify and communicate changes in patient condition.
- Obtain 12-lead ECGs and assist with basic cardiac monitoring as needed; respond to ecg abnormalities per protocol.
- Execute care plans for adults with degenerative disorders (e.g., Alzheimer’s, Parkinson’s) and manage behavioral de-escalation and restraint precautions per policy.
- Maintain accurate electronic charting in EPIC; document nursing care, incidents, and changes in patient status in a timely manner.
- Collaborate with an interdisciplinary team (PT, Radiology, Social Services, Pharmacy) to coordinate care and ensure patient safety; participate in rounding and care conferences as assigned.
- Participate in scheduling and be flexible to float within skillset to meet patient needs; adhere to unit workflows and safety standards.

Requirements
- Active New York single-state LPN license in hand to submit (NY is not a compact license)
- BLS certification
- EPIC charting experience is required
- Minimum 1 year of nursing experience; LTAC and nursing facility/nursing home experience preferred
- Previous charge experience required
- Travel experience required
- Ability to work 5x8 evening shift (1500–2300) and to float within defined skillset
- Weekend requirement: every other weekend
- Holiday availability: every other holiday, if needed
- Education: not specified beyond licensure; candidate must hold an active LPN license in NY
