Job Description
Title: LPN - Nursing Facility

Schedule:
- Shift: 5x8, 1500-2300 (Evening)
- Weekend requirement: Every other weekend
- Holiday requirement: Available every other holiday, if needed

Assignment Details:
- Contract: 13 weeks

Requirements:
- Active New York single-state LPN license required
- Minimum 1 year clinical experience
- Nursing facility/skilled nursing facility experience required
- LTAC experience required
- BLS certification required
- EPIC experience required

Roles & Responsibilities:
- Provide direct patient care to adults and geriatric patients in a skilled nursing facility setting.
- Monitor vital signs and assess pain management needs.
- Implement behavior management and de-escalation techniques.
- Care for patients with degenerative disorders, such as Alzheimer's, ALS, and Parkinson's.
- Obtain 12-lead EKG/ECG as needed.
- Collaborate with an interdisciplinary team including Physical Therapy, Radiology, Social Services, and Pharmacy.

Compliance / Notes:
- Must have active NY LPN license to submit
- Background policy reviewed case-by-case
- Return-staff policy: 1 year
- May require modules pre-start
