Work Environment

This full-time position is primarily onsite at the patients' homes throughout Sacramento, allowing for a flexible yet focused work environment. Our physical therapists collaborate with a diverse range of healthcare professionals, including occupational therapists, nurses, and social workers, to provide comprehensive care plans tailored to each individual. In this collaborative setting, therapists have access to various tools and resources designed to aid in the rehabilitation process.

Key Responsibilities

• Execute daily operations related to delivering physical therapy in a home care environment, which includes managing scheduling, patient documentation, and treatment planning.
• Conduct thorough evaluations of patients to assess their physical condition and rehabilitation needs, creating personalized care plans based on best practices and protocols.
• Document evaluations, progress notes, and reassessments accurately and timely in adherence to industry standards and internal policies.
• Implement effective treatment strategies, utilizing a range of therapeutic techniques and modalities, ensuring patients progress towards their rehabilitation goals.
• Monitor and adapt treatment plans based on patient progress, ensuring ongoing communication with patients and their families regarding their care journey.

Patient Population / Client Focus

Our services cater to a wide range of patient demographics, including the elderly, individuals recovering from surgery or injury, and those living with chronic conditions. The Physical Therapist will work closely with each patient to address their unique rehabilitation challenges, supporting their recovery journey and enhancing their independence. Sensitivity to the emotional and physical needs of patients of various age groups is imperative.

Qualifications

• Completion of a Doctorate in Physical Therapy (DPT) program from an accredited institution.
• An active Physical Therapist license in California, with no history of sanctions or prior board proceedings.
• Exceptional mastery of diverse manual therapy techniques and modalities that enhance treatment outcomes.
• Strong written and verbal communication skills to effectively interact with patients, families, and interdisciplinary team members.

Preferred Experience

• Previous experience in a home care or outpatient rehab setting is preferred, showcasing a strong ability to work autonomously.

• Experience with patient assessments and developing individual therapy plans will be seen as an advantage.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience.
