Work Environment

This role is primarily onsite, focusing on home care, which allows you to engage directly with patients in their familiar environment. Key Responsibilities

• Conduct thorough patient assessments to determine individual therapy needs and develop tailored treatment plans that focus on rehabilitation goals.
• Implement therapeutic interventions using various techniques, including manual therapy, therapeutic exercises, and education on adaptive equipment.
• Monitor and document patient progress, adjusting care plans as needed to meet rehabilitation goals.
• Instruct patients, their families, and caregivers on effective exercises, therapies, and the usage of any prescribed therapeutic appliances or prosthetics.
• Collaborate with multidisciplinary teams, including physicians and nursing staff, to ensure integrated and coordinated care for your patients.
• Utilize electronic medical record (EMR) systems for documentation, scheduling, and monitoring patient progress effectively.
• Participate in ongoing education and training initiatives to stay current with industry trends and best practices.

Patient Population / Client Focus

The patients served in this role will include a wide range of clientele, from pediatric to geriatric populations. Commonly treated conditions may include post-surgical rehabilitation, orthopedic injuries, neurological impairments, and chronic pain management. Your ability to personalize each patient's care will be key to improving their functional outcomes and enhancing their overall well-being.

Qualifications

• A valid license as a Physical Therapist in California.
• A Doctor of Physical Therapy (DPT) degree is required.
• Certification in Basic Cardiac Life Support (BCLS) must be obtained within 30 days of employment.
• Strong communication and interpersonal skills for effective collaboration with patients, families, and team members.
• Commitment to delivering compassionate care and fostering positive patient interactions.

Preferred Experience

• Prior clinical experience in a home care setting is preferred but not required.
• Familiarity with EMR systems and telehealth platforms is advantageous.
• Any specialty certification in areas such as orthopedics or geriatrics can be an asset.

Compensation and Benefits

This role offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience.
