Key Responsibilities\n

• Conduct thorough assessments to identify the unique needs of each client, ensuring tailored treatment plans that promote effective mental health outcomes.

• Maintain consistent communication with supervisors, family members, and other relevant parties to discuss client progress and address any emerging concerns.

• Develop and present a structured schedule for client appointments, home visits, and observations necessary for monitoring individual progress.

• Coordinate required home visits, performing in-home observations, conducting chart reviews, collaborating with staff, and analyzing data to optimize service delivery.

• Implement program plans effectively, ensuring high standards for care delivery and client satisfaction

• Participate in interdisciplinary team meetings and additional discussions to streamline care and share best practices among providers.

• Prepare monthly and quarterly progress evaluations that document client developments and signal necessary adjustments in therapeutic approaches.

• Organize and facilitate training sessions for team members to ensure adherence to program protocols and documentation standards.
\n
• Provide ongoing support through follow-up interviews, home visits, and observations to address the evolving needs of clients.

• Maintain organized and accessible records that serve the needs of clients, staff, and regulatory requirements.

• Act as a liaison within the community and public mental health circles, fostering relationships that aid in achieving developmental goals for clients.

Patient Population / Client Focus\n

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is essential.
