Key Responsibilities\n

• Conduct comprehensive evaluations to assess the needs and treatment goals of clients, utilizing a strengths-based approach.

• Facilitate clear communication between clients, their families, and other healthcare professionals, providing updates on progress and collaborating on care strategies.

• Develop, implement, and monitor individualized treatment plans, documenting all progress in electronic medical records (EMR) software.

• Participate in interdisciplinary team meetings to share insights and collaborate on treatment approaches for a diverse range of cases.

• Arrange and conduct home visits as needed to provide in-home therapy or assessment, ensuring a holistic understanding of each client’s circumstances.

• Compile and complete monthly and quarterly progress reports, ensuring compliance with the organization’s standards and state regulations.

• Provide training and ongoing support for junior staff and new team members regarding program execution and documentation procedures.

• Maintain accurate records and documentation to ensure availability and compliance with privacy practices and legal standards.

• Act as an advocate for clients, helping them navigate resources within the mental health community to achieve their personal growth and development goals.

Patient Population / Client Focus\n

This position involves working with a diverse client population, including adults and youth facing various mental health challenges. You will engage with individuals seeking help for anxiety, depression, trauma, relationship issues, substance abuse, and more. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.
