Key Responsibilities\n

As an LCSW at Trillium Health Center, your primary duties will include:

• Working directly with clients suffering from major mental illnesses and severe emotional disorders, both in virtual and community settings.
\n
• Participating actively in team meetings to align on goals, share best practices, and develop strategies that enhance the quality of care.
\n
• Creating and implementing integrated care plans that ensure client access to necessary services, while maintaining communication with clients, family members, and other healthcare providers.
\n
• Conducting thorough assessments and evidence-based screenings to tailor treatments effectively, arranging for detailed evaluations when necessary.
\n
• Delivering individual, group, and family therapy focused on the present needs and requests of clients, ensuring that therapy is rooted in choice and empowerment.

• Providing crisis intervention as needed, ensuring a safe and supportive environment for clients.
\n
• Monitoring client progress through regular evaluations and coordinating their admission and discharge processes.
\n
• Completing all required documentation and reports promptly, as well as being available for on-call duties as assigned.

This position primarily serves clients from diverse backgrounds who are dealing with mental health challenges, many of whom have limited access to traditional healthcare resources. Qualifications\n

To be successful in this role, candidates should meet the following qualifications:

• A Master's degree in Social Work (MSW) or a related field is required.

• Active LCSW licensure in the state of Connecticut.

• Expertise in assessment, diagnosis, treatment planning, and crisis management, particularly for mental health populations.

• Proven organization and planning abilities to manage caseload and maintain compliance with reporting requirements.

Preferred Experience\n

While a minimum of 2 years of relevant clinical experience is required, candidates with extensive backgrounds in mental health treatment, particularly in detox and community health settings, are preferred.
