• Duration: 13-week contract
• Schedule: Monday through Friday, 8:00 AM - 5:00 PM
• Shifts per Week: 5
• Shift Duration: 8 Hour Days
• Hours per Week: 40
• Setting: Acute care case management leadership
• Call Expectations: After-hours support as needed with 24/7 leadership accountability
• EMR: Cerner

Job Requirements

• Active RN license in Indiana
• Bachelor's degree required; Master's degree preferred
• 4-6 years of nursing experience in a healthcare setting
• 1-3 years of leadership or nursing management experience
• 3-5 years of acute care case management experience
• Strong knowledge of regulatory standards , discharge planning , utilization review , and care coordination
• Ability to lead interdisciplinary teams and collaborate with physicians, staff, and community partners
• Experience with electronic medical records, preferably Cerner

Responsibilities

• Oversee the implementation, development, and ongoing management of care management and resource management programs
• Ensure compliance with regulatory, accreditation, and organizational standards
• Provide leadership, direction, and accountability to case management staff
• Review charts to assess severity of illness, intensity of service, admissions, continued stays, and discharge planning needs
• Escalate cases that do not meet criteria to the appropriate physician advisor or utilization management committee
• Identify avoidable days and implement communication strategies to improve throughput and patient flow
• Coordinate discharge planning and social services functions in accordance with federal and state requirements
• Serve as a liaison with community agencies to support post-acute placement and ongoing patient resources
• Participate in patient care conferences and length-of-stay meetings to support quality outcomes and operational efficiency
• Promote a patient-centered approach while supporting safe, timely, and effective transitions of care
