Geriatric Nurse Practitioner - Primary Care & Home Visits

PACE Southeast Michigan

PACE Southeast Michgan seeks a Nurse Practitioner to provide primary care for an assigned participant panel, under supervision of the center PIC. Responsibilities include chronic disease management, quarterly assessments, and care coordination across the interdisciplinary team. The NP will perform prescribed diagnostics, collaborate with physicians, and support end-of-life discussions within the PACE framework.
