Work Environment\n

This full-time Clinical Social Worker position is primarily remote, allowing for flexibility in managing your work schedule. Regular hours are Monday to Friday from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend tasks based on client needs. You will work closely with our interdisciplinary team via telehealth platforms, ensuring ongoing care and support, and may be required to be on-call to address urgent client situations.

Key Responsibilities\n

Your primary responsibilities will include:

• Conducting comprehensive assessments to evaluate the psychosocial needs of seniors, taking into account emotional, mental, and social factors.

• Collaborating with clients and their families to develop tailored care plans that address individual goals and preferences.

• Providing professional counseling and emotional support to seniors and their families as they navigate transitions and challenges.

• Advocating for clients' rights and helping them access community resources to ensure they receive appropriate support and services.

• Coordinating with healthcare professionals, such as nurses and caregivers, to enhance the overall quality of care provided.

• Monitoring clients' well-being through regular follow-ups, including home visit assessments of their living conditions.

• Facilitating communication among family members to resolve conflicts and ensure cohesive care delivery.

• Educating clients and their families about available resources, support groups, and programs that promote social engagement and improve quality of life.

Qualifications\n

Compensation and Benefits\n

This rewarding role offers a competitive salary ranging from USD 60,000 to 100,000 per year, commensurate with experience.
