This full-time role focuses on providing essential psychosocial support services to senior patients within our hospice center, helping them navigate the complex emotional and social challenges that come with end-of-life care.

Work Environment\n

This role is performed primarily in a remote setting, allowing for flexible scheduling while ensuring that our clients receive uninterrupted support. You will be expected to conduct home visits and coordinate care plans effectively, emphasizing the importance of face-to-face interactions with seniors and their families while providing ongoing communication and collaboration through video conferencing and other digital tools. Our working hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evenings or weekends as needed.

Key Responsibilities\n

• Perform comprehensive psychosocial assessments to identify the emotional, mental, and social needs of senior clients, ensuring a thorough understanding of their unique circumstances.

• Develop, implement, and periodically review personalized care plans in collaboration with clients and their families, ensuring alignment with their goals and preferences.

• Provide individual counseling and emotional support, helping clients navigate significant life transitions and family dynamics, particularly in hospice situations.

• Act as an advocate for clients, ensuring their rights are upheld and that they gain access to necessary resources and services within the community.

• Work closely with interdisciplinary teams, including nurses, caregivers, and physicians, to create a cohesive care experience for seniors and their families.

• Conduct regular home visits to monitor clients' emotional well-being and living conditions, addressing any urgent concerns or safety issues during these interactions.

• Facilitate family meetings to enhance communication, resolve conflicts over care decisions, and provide support to family members as they cope with their loved ones' conditions.

• Educate clients and families about community resources, support groups, and available programs that can enhance social engagement during this critical time.

• Maintain detailed and accurate documentation of assessments, care plans, treatment progress, and interactions to ensure a high standard of care and compliance with regulatory requirements.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution recognized by the Council on Social Work Education.

• Current and valid licensure as a Licensed Master Social Worker (LMSW) in New Jersey.

• A minimum of two years of relevant social work experience, preferably within a geriatric or hospice care setting.

• Strong familiarity with social work principles, elderly care dynamics, and local community resources that can assist seniors.

Compensation and Benefits\n

The salary for the Clinical Social Worker position ranges from $60,000 to $100,000 per year, commensurate with experience and qualifications.
