Key Responsibilities\n

• Conduct comprehensive assessments to thoroughly understand each client’s social, emotional, and psychological needs related to end-of-life care.

• Provide individual and group counseling services using evidence-based therapeutic approaches tailored to the unique challenges faced by clients in hospice care.

• Develop, implement, and regularly reassess individualized service plans that align with clients’ goals and objectives to ensure optimal support throughout their hospice journey.

• Advocate passionately for clients' rights, ensuring they have access to the resources and support they need as they navigate their circumstances.

• Engage in program development, evaluation, and grant reporting to enhance the quality and reach of our services.

• Collaborate with community organizations, healthcare providers, and government agencies to strengthen service delivery and address healthcare disparities.

• Participate in multidisciplinary team meetings, training sessions, and community outreach events to promote awareness and education regarding hospice care and social justice.

Patient Population / Client Focus\n

Your role will primarily focus on providing emotional and practical support to terminally ill patients and their families. You will help families navigate the complex emotions and decisions associated with end-of-life care while offering resources to ensure they can live their remaining days with dignity and comfort.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• In-depth understanding of social justice issues, advocacy, and trauma-informed care approaches.

• Excellent communication, empathy, and organizational skills to effectively engage with clients and families during sensitive situations.

Preferred Experience\n

Compensation and Benefits\n

The position offers a competitive salary ranging from USD 75,000 to 100,000 annually based on experience and qualifications.
