This full-time, remote position allows you to contribute to patient care from the comfort of your own space while collaborating closely with a multidisciplinary team of healthcare professionals. Key Responsibilities\n

• Conduct thorough mental health assessments for clients, addressing personal histories, presenting concerns, and unique risk factors associated with terminal illnesses.

• Design and implement individualized treatment plans that cater to the emotional and mental health needs of patients and their families, advocating for mental wellness throughout their hospice journey.

• Deliver evidence-based therapeutic interventions, such as Cognitive Behavioral Therapy (CBT) and trauma-informed approaches, tailored specifically for individuals facing end-of-life issues.

• Facilitate both individual and group therapy sessions to promote emotional expression, coping mechanisms, and personal growth, ensuring a supportive environment for patients and their loved ones.

• Collaborate with physicians, nurses, chaplains, and other mental health professionals to ensure a coordinated approach to care, addressing both physical and emotional health concerns.

• Provide timely crisis intervention and manage mental health emergencies with sensitivity, employing de-escalation techniques and making appropriate referral recommendations as necessary.

• Engage in psychoeducation for clients and their families, assisting them in understanding mental health conditions and available treatment options, thus empowering them through knowledge.

• Maintain diligent documentation of assessments, treatment plans, and progress notes, ensuring compliance with ethical and legal standards relevant to medical and mental health documentation.

• Participate in virtual meetings and contribute to interdisciplinary case discussions, fostering a culture of collaboration and continuous improvement.

• Stay informed on current legislation and ethical guidelines governing mental health practices, particularly in hospice settings to promote best practices.

Patient Population / Client Focus\n

Your primary clients will be individuals within our hospice center, primarily adults dealing with terminal illnesses. Patients will often experience a range of emotional responses, including anxiety, depression, and grief, necessitating a tailored approach aligned with their unique experiences and needs. Engaging with families is also a critical component of supporting the overall mental health of the patient’s support system.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• A valid and current LCSW license in the state of New Jersey.

• A minimum of two years of post-licensure experience is required, particularly in a mental health or healthcare setting; hospice experience is a significant plus.

• A solid understanding of mental health disorders, therapeutic interventions, and the complexities of trauma-informed care relevant to end-of-life scenarios.

• Experience in conducting mental health assessments and developing actionable and individualized treatment plans.

Preferred Experience\n

Previous experience in hospice care or palliative settings is highly advantageous. Familiarity with community resources for terminal patients and their families and a strong commitment to advocacy and compassionate care will set candidates apart. Additional certifications in specialized therapeutic interventions, such as grief counseling or trauma-informed care, are desirable.

Compensation and Benefits\n

We offer a competitive salary range of $80,000 to $100,000 annually, commensurate with experience and qualifications.
