This position operates in a unique remote work setting primarily based at our Hospice Center. You will be expected to engage with clients virtually, applying your clinical expertise in a supportive environment crafted to address the holistic needs of individuals. Key Responsibilities\n

• Provide direct clinical support to clients in both office and community settings, addressing symptoms of major mental illness and severe emotional disorders.

• Engage in collaborative treatment planning, ensuring that care plans are tailored to the specific needs and circumstances of each client.

• Conduct evidence-based screenings and assessments to determine appropriate levels of care and intervention, followed by detailed reporting.
\n
• Implement and monitor integrated care plans, facilitating communication among clients, family members, and other health providers to optimize outcomes.
\n
• Deliver a spectrum of brief, focused treatments and recovery services, employing therapeutic techniques including individual, group, and family therapy as needed.
\n
• Participate in crisis intervention efforts as necessary, providing timely support and resource referrals to clients in distress.
\n
• Maintain accurate records and documentation to ensure compliance with all regulatory standards and agency policies.
\n
• Engage in ongoing professional development activities to refine skills and enhance service delivery.

• Take part in the center’s quality improvement initiatives, offering insights to enhance overall service provision.

Patient Population / Client Focus\n

In this role, you will primarily serve clients from diverse backgrounds who are facing chronic mental health conditions and serious emotional challenges. Your ability to understand and respect different traditions and beliefs is crucial as you work collaboratively to develop meaningful connections with clients. Qualifications\n

• A Bachelor’s degree in Social Work is required; a Master’s degree is strongly preferred.

• Must possess a valid LCSW license allowing practice in New Jersey.

• Demonstrated commitment to professional ethics and the ability to maintain confidentiality.

• Excellent verbal and written communication skills to effectively engage with clients and team members.

• Thorough knowledge of mental health assessment, diagnosis, and treatment methodologies.

Preferred Experience\n

• At least two years of relevant direct clinical experience in a similar setting is preferred.

Compensation and Benefits\n

The salary range for this position is $70,000 to $85,000 annually, commensurate with experience.
