Key Responsibilities\n

• Conduct thorough assessments of clients’ social, emotional, and environmental challenges, utilizing established frameworks to gather and evaluate information effectively.

• Develop and implement comprehensive service plans tailored to individual needs, incorporating specific goals, intervention strategies, and methods for monitoring progress.

• Provide individual and family counselling services, utilizing evidence-based therapies to support those navigating complex life issues.

• Engage collaboratively with clients to facilitate access to necessary resources and support services within the community.

• Implement crisis intervention strategies and conduct risk assessments, crafting safety plans when necessary to ensure client well-being.

• Work alongside community agencies and organizations, promoting coordinated service delivery and advocating for the rights and needs of clients.

• Participate in community outreach efforts to enhance awareness of available services and build relationships throughout the community.

• Contribute to case conferences, team meetings, and ongoing training, promoting a culture of knowledge sharing and professional development.

Patient Population / Client Focus\n

In your capacity as an LCSW, you will primarily serve individuals and families facing significant social challenges and health disparities. This includes clients experiencing mental and emotional distress, homelessness, domestic abuse, and addiction. • A Master’s Degree in Social Work from an accredited institution is required.
