Key Responsibilities\n

• Conduct psychosocial assessments through interviews and thorough analysis of medical records to identify factors affecting a patient’s recovery process.

• Provide empathetic counseling and emotional support to patients and their families, assisting them in managing adjustment issues and emotional distress related to recovery.

• Collaborate with the rehabilitation team to create and implement personalized treatment plans that address individual psychosocial needs.

• Help patients gain access to community resources, such as vocational rehabilitation services, support groups, and housing assistance programs.

• Educate and support patients and their families regarding available community resources and post-rehabilitation services available after discharge.
\n
• Take an active role in discharge planning to ensure smooth transitions to home or alternative care settings, aiming for sustained recovery outcomes.
