Key Responsibilities\n

• Conduct comprehensive clinical evaluations, including biopsychosocial assessments, to develop individualized treatment and aftercare plans for hospice patients and their families.

• Provide counseling services tailored to patients' emotional and psychological needs, helping them navigate the complexities of end-of-life care.
\n
• Collaborate closely with physicians, nursing staff, and other healthcare professionals to monitor treatment efficacy and medication side effects, ensuring holistic patient care.

• Engage in case management activities, including participating in interdisciplinary treatment team meetings and conducting utilization reviews to optimize care pathways.
\n
• Document patient progress and treatment plans meticulously, including crisis intervention notes, discharge planning, and session records, in accordance with company policies.
\n
• Research and advocate for community resources to assist patients and families in accessing necessary support services such as financial aid, counseling, and healthcare services.

• Facilitate educational workshops for families and patients to inform them about available resources and coping strategies during challenging times.

Patient Population / Client Focus\n

In this role, you will primarily serve patients in hospice care, along with their family members and primary caregivers. This patient population often grapples with complex emotional and psychological challenges associated with terminal illnesses. As a key member of the care team, your role is to provide empathetic support and practical guidance to navigate these difficult experiences, ultimately enhancing their quality of life during this sensitive time.

Qualifications\n

• A Master’s Degree in Social Work from an accredited program.

• Possession of an active and unrestricted Licensed Clinical Social Worker (LCSW) license.

• A minimum of 2 years of clinical experience in a behavioral health or healthcare setting, specifically related to case management.

• Demonstrated knowledge of end-of-life issues, loss and grief processes, and ethical decision-making in clinical social work.

• Strong crisis intervention skills and experience conducting psychosocial evaluations, including suicide assessments.
\n
• Excellent interpersonal skills, with the ability to manage conflict and build rapport with patients and team members.

Preferred Experience\n

• Experience in hospice or palliative care settings is highly desirable.

• Familiarity with telehealth platforms and documentation software.

• Training in trauma-informed care practices.
\n

Compensation and Benefits\n

The salary range for this position is USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
