Key Responsibilities\n

• Conduct in-depth psychosocial assessments via telehealth appointments, utilizing interviews, observation, and medical record reviews to evaluate patients’ emotional, social, and environmental factors impacting their health and recovery.

• Provide essential counseling and support to patients and their families, focusing on adjustment issues, emotional well-being, and coping mechanisms related to their conditions.

• Collaborate closely with an interdisciplinary rehabilitation team to design and implement tailored treatment plans that consider the psychosocial needs of each patient.

• Assist patients in accessing community resources, such as support groups, vocational rehabilitation services, and housing assistance, to further facilitate their recovery process.

• Educate both patients and their families on available resources and post-rehabilitation services, ensuring smooth transitions and continued support after discharge.

• Engage in discharge planning activities, facilitating transitions to home or alternative care settings and enhancing overall patient outcomes.
